The MRCEM Primary syllabus: six subjects and how they're weighted
A practical breakdown for planning your revision. Always confirm the current blueprint against the official RCEM documents before you rely on it.
The MRCEM Primary tests the basic sciences that underpin emergency medicine, and it does so across six subjects. Understanding what those subjects are — and, crucially, how much of the paper each one represents — is the single most useful thing you can do before you start revising. It stops you spreading your time evenly across subjects that are not weighted evenly.
Where the syllabus comes from
The MRCEM Primary is mapped to the RCEM Basic Sciences Curriculum, which is the authoritative document defining what is examined. RCEM strongly advises all candidates to familiarise themselves with it before sitting the exam. It sets out, subject by subject, exactly what you are expected to know — and, importantly, it often states where detail is not required, which can save you a lot of wasted revision time. The exam blueprint and the question split are published in the official MRCEM Primary Regulations and Information Pack on the RCEM website.
The six subjects and their approximate weighting
The 180 questions are divided across the six subjects, and the split is heavily weighted towards anatomy and physiology. The widely-cited breakdown is:
- Anatomy — ~60 questions. The joint-largest subject.
- Physiology — ~60 questions. The other joint-largest subject.
- Pharmacology — ~24 questions.
- Microbiology — ~17 questions.
- Evidence-based medicine — ~10 questions.
- Pathology — ~9 questions.

The headline message is unmissable: anatomy and physiology together account for roughly two-thirds of the entire paper. If you are strong in those two subjects, you have a very large safety margin. If you are weak in either, it becomes extremely hard to pass no matter how well you do elsewhere. Plan your time accordingly.
What each subject actually covers
Anatomy
The RCEM Basic Sciences Curriculum covers regional and systemic anatomy — including the limbs, thorax, abdomen, head and neck, and the nervous system. In the exam, questions rarely ask simply “where is this structure?”. They tend to ask what happens when a structure is injured, compressed or compromised — so understanding clinically relevant relationships (for example, which nerve is vulnerable at a particular site) matters more than rote memorisation of every muscle.
Physiology
The physiology curriculum spans basic cellular physiology, respiratory, cardiovascular, gastrointestinal, renal and endocrine physiology. Questions test mechanistic understanding — given a physiological principle, what would happen if something changed — rather than isolated facts.
Pharmacology
Pharmacology covers general principles (how drugs are absorbed, distributed, metabolised and eliminated, and how they act) as well as the major drug classes relevant to emergency medicine. Understanding mechanisms and side-effect profiles is more valuable than memorising long drug lists.
Microbiology
Microbiology covers the principles of infection and the organisms relevant to the emergency department — bacteriology, virology, and related topics including antimicrobial principles and infection control.
Pathology
Pathology focuses on the mechanisms of disease — cellular injury, inflammation, wound healing, and related processes. Although it carries relatively few marks, the concepts often integrate with other subjects.
Evidence-based medicine (EBM)
EBM covers study design, basic statistics (such as sensitivity, specificity and number needed to treat) and critical appraisal. Its curriculum is short, but it reliably carries around 10 marks — so it is worth learning early and properly rather than leaving it as a last-minute panic. It is arguably the best return on investment in the whole syllabus.
How to use the weighting in your revision
A sensible approach is to protect your time in anatomy and physiology first, because that is where most of the marks are. Front-load EBM early since it is small and high-value. Then ensure solid coverage of pharmacology, microbiology and pathology — the marks there are still marks, and they often make the difference around the pass mark. Because questions frequently integrate more than one subject in a single stem, understanding principles across subjects pays off more than siloed memorisation.
